Information we collect
Appointment requests collect your name, email, phone number, state or country, preferred format and time, and consent choices. General inquiries collect a name, email and message. We store request timestamps and the version of the appointment notice accepted. A short-lived keyed hash of the requesting IP address helps limit abuse; hosting providers may keep separate access logs. The scheduling assistant uses fixed rules in your browser and does not send chat text to an AI provider or our server.
How we use it
We use contact and scheduling information to answer inquiries, assess appointment eligibility, arrange appointments, send operational updates and protect the website. Where applicable, the legal basis is taking steps at your request before a service, operating and securing the website in our legitimate interests, meeting legal obligations, or consent for optional contact channels. We do not use appointment data for advertising audiences or sell it.
Contact choices
Operational appointment email is part of the requested scheduling service. Phone calls and SMS are separate optional choices, not conditions of requesting an appointment. These choices do not authorize advertising, automated marketing calls or prerecorded messages. You can change them using your private appointment key or by contacting the office. Message or data charges may apply if you choose SMS. This website does not itself send SMS.
Service providers and transfers
Hosting and configured email delivery providers process information as necessary to operate the site. The office must identify and assess its chosen providers before launch. Choosing WhatsApp, Google Translate, a map, a social profile or an external video connects you to that provider under its own policies. Providers may process information in other countries. External video is blocked until you enable it. Do not put medical records or sensitive health information in these services.
Retention and security
Access to scheduling records is restricted to authenticated administrators. The website supports deletion of records by the office; it does not automatically delete clinical records or set a clinical retention period. The office must set a retention schedule consistent with its obligations and delete website inquiries when no longer needed. Backups and hosting logs may follow separate retention schedules. HTTPS, protected storage, access controls and secure email configuration are required for deployment; no system can promise absolute security.
Your choices and rights
Depending on your location, you may be able to request access, correction, deletion, restriction or portability, object to certain processing, or withdraw consent. Contact the office so it can verify your identity and respond under applicable law. You may also contact your local privacy regulator. Withdrawing consent does not affect earlier lawful use. Browser Global Privacy Control is honored by keeping optional external media off by default.
Children and health information
Online booking is intended for adults aged 18 or older, including adults authorized to arrange care for another person. This is not a clinical records portal. Do not upload or enter genetic results, diagnoses, insurance details or identity documents. If such information is needed, the office will arrange an appropriate channel. This notice is not a declaration that the website has been independently certified for HIPAA or other regulatory compliance.
Changes and questions
The policy version accepted at booking is stored with the request. Material changes will be reflected on this page. Contact the office with questions about data use or to withdraw optional contact permission.
